MCHoose wins at Catholic Health Week Awards

Catholic Health Australia National Conference 2022

Meeting the needs of patients with limited English and poor understanding of Australia’s fragmented healthcare system is not easy. Collaboration between Inala Primary Care, Mater Health Services, Footprints and Brisbane South PHN generated a model of care embedded in general practice specifically for migrant populations.

The team of care spanned GPs, nurses and social workers and delivered social prescribing and care coordination.  MCHoose was evaluated across five sites.  It delivered better outcomes for patients and their care providers. The effort was recognised at the national Catholic Health Australia conference awards night.
