- AGPAL Australian Practice of the Year 2013 – 2014/15, awarded May 2016
- Lowering costs, localising care and improving patient outcomes in the areas of diabetes, CKD and refugee health
- Tripling the number of doctors working from the practice and regularly receiving inquiries about working at the practice from both potential registrars and qualified GPs
- Gaining long-term contracts valued at in excess of $1M and over $2M in pilot funding for novel models of chronic disease care in the last 3 years
- Translating the novel models of care into three other sites with more on the drawing board
- Uploading the first national electronic health record
- Consulting to the Hong Kong Hospital Authority
- Having staff nominated for national committees
- Breezing through accreditation for both our teaching and general practice work
- Having two staff inducted as Angels by Kidney Health Australia
- Having at least one article in the newspaper about our practice for each of the last four years
- Our CEO being awarded a Winston Churchill Memorial Trust fellowship
- Delivered 40,000 appointments in the 2017 calendar year, growth of 19%
- January 2018 hosted a Tasmanian delegation of seven interested in complex care
- January 2018 began “My Health for Life” chronic disease prevention workshops
- February to April 2018 hosted 5 nurses from Hong Kong
